I understand if one or more of the following conditions occur, it will constitute a voluntary resignation/quit from my employment with TESI/AGCI: Failure to call TESI/AGCI at the end of each assignment (regardless of the reason for separation from the assignment) to notify TESI/AGCI of my availability; Failure to call TESI/AGCI on a weekly basis to notify TESI/AGCI of my availability; Failure to notify TESI/AGCI of a change in my address and/or phone number; Failure to accept a suitable work assignment due to pay, qualification or location; TESI/AGCI's receipt of an unemployment claim from me without prior notification of my availability."
